Where the death is registered

  • Death at home: the registrar of that area: gram panchayat in villages, or the municipality, council or corporation in towns. The head of the family or nearest relative must inform them within 21 days.
  • Death in a government hospital: the hospital's officer-in-charge, who is the sub-registrar, registers it.
  • Death in a private hospital: the registrar of the area where the hospital is.

Proof of the death (e-Mitra applications)

Details to check carefully

  • The deceased's name, date of death, parents' names, and the husband's or wife's name if married.
  • Spellings should match the deceased's Aadhaar and Jan Aadhaar; banks and insurers compare them.
  • Correcting a registered certificate later takes another application, so check the form before you submit it.

Registering late

  • 21 to 30 days: registered with a small late fee.
  • 30 days to 1 year: written permission from the block nodal officer (rural) or district registrar (urban), with an affidavit.
  • More than 1 year: permission from an executive magistrate (Collector, SDO or Tehsildar).

For permission, keep ready: your affidavit in two copies with a photo, your ID, the deceased's Aadhaar or other ID, and witnesses' affidavits with their ID. Confirm the current procedure with the registrar before you prepare papers.
